Community Connections Program

The Community Connections Program (CCP) provides assistance to people who experiencing or at risk of homelessness or live in low quality accommodation and have multiple unmet health needs.

The Community Connections Program is funded by Home and Community Care (HACC) to undertake work with adults within the local government areas of Hume and Moreland.

The program aims to provide assistance to people who are experiencing homelessness or are at risk of homelessness or who live in low quality accommodation and have multiple unmet health needs.

The program uses an assertive outreach approach to locate and engage clients. CCP provides short term case management to assist clients to access mainstream and/or specialist health services and aims to improve social connectedness for the person depending on their needs.

CCP aims to assist clients to develop an independent and sustainable support network in the community of their choice.

Eligiblity

To be eligible for the Community Connections Program you must be:

  • Aged 25 years and over
  • Experiencing or at risk of homelessness
  • Living in low quality or insecure accommodation
  • Have multiple unmet health needs including:
    • Intellectual and/or physical disability
    • Mental illness
    • Acquired brain injury
    • Age related or premature frailty
    • Chronic drug or alcohol dependence
    • Chronic social isolation

Referrals

You can ring the Community Connection Program staff to refer yourself. Someone else is also able to call on your behalf; family, friend, doctor or another person.

Service providers/support agencies are also encouraged to contact the CCP workers to seek further information regarding eligibility and the referral process. Eligible individuals can be referred directly from local Government agencies, health services, hospitals, Centrelink, Office of Housing, etc.

The CCP service is a free and entirely voluntary and clients may withdraw from the program at any time.
